Daniel Hodgkiss

Title: PIER – Focusing on Identification and Early Escalation of soft signs of deterioration in whatever healthcare setting

Background – Daniel leads the Deterioration NatSIP for the West Midlands focusing on acutely ill patient deterioration/ implementation of NEWS2 (National Early Warning Score); and the spread and adoption of the Bristol ED Checklist. Daniel worked previously for Walsall Healthcare NHS Trust from 2014 – 2019 and was the Patient Safety Manager for MLTC and Adult Community, where he investigated serious incidents ensuring Human Factors were an integral aspect of all investigations. Daniel developed a nationally recognised initiative relating to the management of frequent attenders to A&E who had complex social issues. Daniel embedded Appreciative Inquiry throughout the Trust developing a number of approaches and tools such as Right Cause Analysis and delivered a workshop at the World Appreciative Inquiry Conference 2019 surrounding the movement to a Safety-II culture.  Daniel was part of the 2018 HSJ winning team for the Clinical Governance and Risk Management Award and the Patient Safety Meridian Award in 2019.  Daniel has a real passion for developing positive safety cultures in healthcare using Quality Improvement and Appreciative Inquiry approaches whilst ensuring patients are at the heart of the work.
